The checklist below will speed the process in enrolling at North Davidson Middle School. You MUST have this information in order to complete the enrollment process.

Birth Certificate for new student

Social Security Card for new student
-
TWO proofs of residency with the parent/legal guardian's name proving you reside in the NDMS district
-
Examples: voter registration, tax listing, postal change
verification, telephone bill, utility bill, rental/lease agreement,
vehicle registration card, etc.

Most recent custody papers (if applicable)

All personal information including emergency phone contacts, healthcare providers, etc.

Copy of most recent report card
-
Most recent standardized test score
-
North Carolina EOG, California Achievement Test, Otis-Lennon, etc.

A copy of any special program paperwork where applicable

AIG - original placement scores: aptitude and achievement
-
IEP - most recent plan
-
504 Plan - photocopy of modifications for local review

Bring
COMPLETED application packet. Do NOT fill out the Notary Section
at the bottom of page 1. You must bring a driver's license or
picture ID to have the application notarized.
Very Important! The registration process MUST BE
COMPLETED BY A BIOLOGICAL PARENT or LEGAL GUARDIAN! This does not
include non-custodial stepparents, grandparents, aunts, uncles,
cousins, or siblings!
