YEARBOOK DEDICATION INFORMATION
Fifth graders may have yearbook dedications made in their honor. Checks should be made out to Friedberg Elementary School. PLEASE NO MORE THAN TWO (2) PICTURES! You can write your dedication below and send this form in with a check and the picture(s) to our office OR you can send an email to mzarate1@davidson.k12.nc.us and attach the pictures as jpgs, then send payment into the office. Please put YEARBOOK DEDICATION in the subject heading of the email.
Please put the form, pictures, and check in an envelope and send to the office OR Email Melinda Zarate with your dedication and pictures and send in the check to the office no later than Monday, December 5, 2011. No dedications will be taken after this date!!
