This form is used to request a change in location of a current assigned bus stop.
Please submit completed form(s) to your students home school, they will forward it to the Transportation Department for review.
Forms must be received from the students home school, forms sent directly to the Transportation Department will not be processed. This is a safety precaution to ensure that the forms are actually submitted by the parent/legal guardian.
